Now you can fill out some of our forms online. No more handwritten versions!

Here's how to do it:

1. Click the link for the desired form to open it in Adobe Reader inside your browser window.
2. In Internet Explorer and Firefox, click "Highlight Existing Fields" in the righthand corner of the PDF window. This will highlight all the fields that you can fill out. (Google Chrome users should see the highlighted fields automatically.)
3. Click on each desired field and type your information. Use the enter/return key to type multiple lines in one section and the tab key to advance to the next field.
4. Click the printer button in the Adobe Reader window to print your form.

Please note:

• You will need a FREE Internet application called Adobe Reader (version 7.0 or higher) to do this. Visit Adobe's site to get the free application and instructions on how to use it.
• You cannot save your typed information in the form. Once you close the form, the information you typed in will be deleted.
