Filling Out Programmable Forms

Now you can fill out some of our forms online. No more handwritten versions!

Here's how to do it:

  1. Click the "print" link under the desired form to open the form in Adobe Reader.
  2. Make sure the picture of the hand is selected from the menu at the top of the page.
  3. Place the hand over the area you wish to type in. You'll notice that the hand changes. Double-click to get a cursor.
  4. Type your information in each desired field. Use the enter/return key to type multiple lines in one section and the tab key to advance to the next field.
  5. Select "print" from the Adobe Reader toolbar.

Please note:

  • You will need a FREE Internet application called Adobe Reader (version 7.0 or higher) to do this. Visit Adobe's site to get the free application and instructions on how to use it.
  • You cannot save your typed information in the form. Once you close the form, the information you typed in will be deleted.
  • If you experience problems with the forms using Firefox visit http://support.mozilla.com/en-US/kb/opening+PDF+files+within+Firefox for help.
